OPEN REGISTRATION
 
CCYHA is now holding open registration session for the 2008/2009 season.  Fees may be mailed in along with all completed registration forms (including USA Hockey registration) or paid on-line using the CCYHA Payment Center below.  

Fundraiser Information
This years fundraiser will be selling raffle tickets for a 4-wheeler auction.  Each player will be asked to sell 5 tickets at $10.00 each.  Tickets will be paid for up front by the player/parent.  The player/parent may then decide to keep the tickets and enter the raffle his/her self or sell the tickets at face value and keep the funds collected as reimbursment.  Ticket will be available at the open registration/skate swap on September 13th and the first weekend of the season, September 20th.

Online payments will have a 3% processing fee associated with them.  To register, please follow the steps below:
Step 1  Register on-line with USA Hockey.  Players will be expected to pay USA Hockey registration fees at the time of registration.  

Please mail a copy of your  USA Hockey registration  with the rest of your forms P.O. Box 201, Meadville, PA 16335.
 This information is Bar Coded and will be skanned into the CCYHA database.
Step 2  Download and print the CCYHA registration forms.  (includes registration, payment plan, code of conduct and consent to treat.)
Step 3  Complete the CCYHA registration forms and mail to CCYHA with your payment.

CCYHA will again accept electronic payments this year through a secure PAYPAL account.  Users making payments do not need a PAYPAL account.  If you are making payments on-line, please print and include a copy of your payment receipt with your registration forms.

Fees may be paid for online using a major credit card or your PAYPAL account.  You may pay for multiple players at a time.  Please note player(s) names in the comment section at the time of check-out.  If you do not have a PAYPAL account, choose the option to continue without logging in.

Payment Plan Information:
CCYHA will be offering an on-line payment plan option for the 2008/2009 season during open registration.  This option consists of 6  payments automatically deducted from a credit card or PAYPAL account on a monthly basis.  Note:  if you choose to register using the online payment plan option, a 3% processessing fee is added into the total cost of registration and a seperate plan must be set up for each player.  This is the only payment plan option available for the 2008/2009 season- Payment coupons are not available.

Refunds or credits associated with electronic payments will also be done electronically.
