CCYHA is now holding open registration session for the 2009/ 2010 season. Fees may be mailed in or paid on-line using the CCYHA Payment Center below.
Fundraiser Information
For this year's main fundraiser, we will sell raffle tickets of $5 each to raffle off a 50 inch Plasma TV worth $2000! Each player is required to sell 10 tickets ($50 worth of tickets). The $50 will be collected at registration or when your receive the tickets. You will get reimbursed by selling them. If you do not want to participate, you can buy out of the fundraiser for $45.
We are also selling tickets to a CCYHA Halloween Party featuring THE EARTHQUAKERS! All CCYHA members are encouraged to come and to sell tickets for the event. We will pull the winner of the TV at the Party.
OPEN REGISTRATION
Step 1 Register on-line with USA Hockey. Players will be expected to pay USA Hockey Registration fees at the time of registration.
Your players USA Hockey number is located on the confirmation page. This number is important. is proof of insurance coverage which is required to play. Please print the USA Hockey registration page and mail or turn in to your division head the first day of practice.
Step 2 Download and print the CCYHA registration forms. (includes code of conduct and consent to treat.) Mail or bring these forms to your first practice and give to your division head. You must have these forms to go on the ice.
Step 3 Complete CCYHA's on-line registration form. CCYHA will again accept electronic payments this year through a secure PAYPAL account. Users making payments do not need a PAYPAL account. If you are making payments on-line, please print and include a copy of your payment receipt with your registration forms.
