CCYHA Registration

CCYHA is preparing to hold open registration for the 2011/ 2012 season. Fees may be mailed in or paid on-line using the CCYHA Payment Center below. Fees are charged according to division and are listed in the Payment Center below.

Fundraiser Information--- We are raffling off a 32G ipad2. $5 a ticket. Winner will be pulled during the Halloween Parade.

OPEN REGISTRATION

Step 1 Register on-line with USA Hockey. Players will be expected to pay USA Hockey Registration fees at the time of registration.

Your players USA Hockey number is located on the confirmation page. This number is important. It is proof of insurance coverage which is required to play. Please print the USA Hockey registration page and mail in with other registration forms.

Step 2 Download and print the CCYHA registration forms . Mail these forms to the address below. You must have these forms to go on the ice.

Step 3 CCYHA will again accept electronic payments this year through a secure PAYPAL account. Users making payments do not need a PAYPAL account. If you are making payments on-line, please print and include a copy of your payment receipt with your registration forms.

CCYHA ON-LINE PAYMENT CENTER

Fees may be paid for online using a major credit card or your PAYPAL account. You may pay for multiple players at a time. If you do not have a PAYPAL account, choose the option to continue without logging in.

Pay in monthly installments using the buttons next to the player's division category. Your final payment will be February 2011. Your monthly payment is calculated corresponding to the month you start your subscription.

A 3% Convience fee will be added to your payment.
